There are a few forms you will need to fill out , including our health history form, which can be done in the office or downloaded (click button above) and completed before your visit.
While you wait, you can relax in our waiting room or sip on a fresh cup of coffee. During your time in the chair, enjoy our flat-screen televisions in every room.
It is important to completely assess your current oral health, because each patient is an individual with a unique health history and dental needs. Your dentist will have an in-depth conversation with you to discover your goals and main concerns.
During your initial exam, we will take X-rays and examine your teeth, gums, and bones that hold the teeth. Your dentist will also perform a clinical exam to check for oral, head, and neck cancer, as well as assess the muscles, ligaments, and joints of your mouth.
X-rays are used to help diagnose problem areas and develop an individualized treatment plans. Once you and your dentist have determined the course of treatment appropriate for you, we will store this information in our computer system and provide you with a copy. We will also provide you with the final proposed treatment plan, presented procedure by procedure. This will also include the financial aspect of treatment, including insurance and patient responsibilities.
